In an effort to ensure complete accuracy of your file before going to print, please review and complete the form below, indicating that you have reviewed all changes and that the proof you have received is accurate and ready for print.

IMPORTANT NOTE: Please check your proof very carefully and advise us of any errors, typos, spelling, grammar, content, graphics, or otherwise.  Although we strive to make all corrections as indicated and error-free as possible, once approval has been given, you will be responsible for all content and subsequent print approval (if applicable). SiteWyze assumes no liability for any errors after final print approval has been provided.

In addition, please note that for print, items will appear brighter on your computer screen compared to the final printed product which will appear darker. If we are reprinting an item, there may be slight colour variance between the reprint and the original.
